VACCINATION VISIT

APPOINTMENT

The HSE requires all vaccinations to fulfill the basic legal criteria for all vaccinations: This include full personal detail: Name, Surname, Address, Tel. Nr, e-mail address to receive vaccination details and PPS number for each person vaccinated. If all of this information is not available you will have to make a telephonic booking for your Swine Flu vaccination.

To make your visit to the Clinic as speedy as possible and to reduce your waiting time as well as that of others please follows the following advice:

  1. Make an online booking, registration and complete the online consent. You will be presented online with all the necessary information and documentation to speed up your visit to our Clinic. The online booking system is very simple and will take you through the booking step-by-step. Visit to the clinic after completing the booking online will take you only 5 minutes to receive the vaccination.

  2. Alternatively you are welcome to call the Clinic: 8678442 and reserve your vaccination. Please remember to bring all of the information requested above along with you. Telephone booking visits may take up to 30 minutes to read the information, complete the consent forms and to receive the vaccination.

  3. You can also visit the Clinic as a walk-in. Please remember to bring all of the information requested above along with you. Walk-in visits may take up to 30 minutes to read the information, complete the consent forms and to receive the vaccination.

YOUR VISIT

  1. If you have booked online please report to reception on arrival. Your online booking and completion of the information and consent procedure will be confirmed. You will go through to the Doctor for the vaccination. You will be given the HSE consent form to sign by the Doctor to confirm that you received the vaccination. This procedure takes around 5 minutes.

  2. If you have made a telephone appointment please report to reception on arrival. You will be given information about the vaccination to read. After reading this you can complete the consent form. This needs to be handed back to Reception. You will go through to the Doctor for the vaccination. You will be given the HSE consent form to sign by the Doctor to confirm that you received the vaccination. This procedure takes around 30 minutes.

PERSONAL CONFIRMATION OF VACCINATION DETAILS FOR YOUR RECORDS

The Arte Novi Clinic offers a unique service by allowing you access to your Personal Health File. We will forward you a link, password and pin number to your Personal Health File using the e-mail address you supplied.

You can access your own file and information on the Arte Novi Website, using your unique password and pin number.

This information would include date of vaccination:

  • Details of the specific Vaccine used,
  • Batch Number,
  • Expiry date and after care notes.
